Health and Safety Incident Reports

Health and Safety incident reports are imperative in order to ensure the documentation of any incident or situation where the health and/or safety of a CUPE 2193 member is threatened.  This could include, but is not limited to, being physically attacked, being verbally attacked, entering an unsafe location, etc.  Please keep in mind that these documents don’t only ensure your own safety but also take into consideration future safety planning for your union brothers and sisters.

Incidents and unsafe conditions happen in the workplace all the time.  We need you to report them to your supervisor with a copy to your local.  EVEN IF NO ONE WAS HURT (near-miss or near-hit), an investigation can prevent a future injury.
